Gonadorelin: research

A short-acting fertility hormone used in tests and specialist pump therapy.

Also called: GnRH, LHRH, gonadorelin acetate, gonadorelin hydrochloride, Factrel, Lutrepulse.

Reproductive axis peptides · Gonadotropin-releasing hormone receptor agonist · Evidence: Approved drug.

At least 51 studies in separate reviews, 35 in women and 16 in men. More reports are listed; twice-weekly shots alongside testosterone therapy lack a controlled fertility trial.

Studies

Gonadorelin dosage

Typical reported dose: 200 mcg every 3.5 days (twice a week)

  • Pituitary response test: SC: 100 mcg once single diagnostic injection, under the skin. Clinician-published. Studied in: Adults undergoing an organized pituitary test. (source)

  • LutrePulse: 15 mcg pulses: 15 mcg per pulse every 90 minutes every 90 minutes by pump, under the skin. Studied in a trial. Studied in: Women with primary amenorrhea and hypogonadotropic hypogonadism. (source)
  • LutrePulse: 20 mcg pulses: 20 mcg per pulse every 90 minutes every 90 minutes by pump, under the skin. Studied in a trial. Studied in: Women with primary amenorrhea and hypogonadotropic hypogonadism. (source)
  • Male CHH: response-guided pump: Start 10 mcg per pulse every 90 minutes every 90 minutes by pump, under the skin. Clinician-published. Studied in: Men with confirmed congenital hypogonadotropic hypogonadism. (source)

Gonadorelin side effects and risks

Reported side effects

  • Pump-site skin allergy: 7/20 (35%) in one male pump cohort; not a bolus test rate., moderate (source)
  • Pain, swelling or itching: UK bolus label: 0.1% to <1%., mild (source)
  • Headache, dizziness or nausea: UK bolus label: 0.01% to <0.1%., mild (source)
  • Allergy or airway spasm: UK label classifies hypersensitivity and bronchospasm as rare., serious (source)
  • IV-line infection or vein inflammation: Historical IV line complications averaged 7%; includes more than infection., serious (source)
  • Multiple pregnancy or ovarian over-response: 12% multiple pregnancy in the 1990 program; 1.6% twins in the 2022 cohort., serious (source)
  • Pituitary bleeding in a tumor: unknown, serious. Sudden severe headache or vision loss needs emergency assessment. (source)

Stop and get help if

  • Sudden severe headache, vision change, vomiting or collapse: Seek emergency care for possible pituitary bleeding.
  • Throat swelling, wheeze or faintness after injection: Seek emergency care for possible severe allergy.
  • Fever, spreading redness or chills with a pump or IV line: Get urgent assessment for possible infection.
  • Severe pelvic pain, rapid weight gain or breathlessness: Get urgent assessment for ovarian over-response.

Who should avoid it

  • pregnant: UK product information says not to administer during pregnancy.
  • breastfeeding: UK product information says not to administer while nursing.
  • tryingToConceive: Specialist fertility use requires diagnosis and monitoring.
  • under18: Pediatric research is not an adult self-use regimen.
  • hormoneSensitiveCancer: Precaution: gonadorelin raises sex hormones; breast or prostate cancer requires an oncology plan.
  • other: GnRH can trigger bleeding into a pituitary adenoma.
  • other: Prior allergy to gonadorelin or its excipients can recur.
  • other: Primary gonadal failure or poor pituitary reserve can limit response; diagnose the cause first.
  • testedAthlete: Prohibited at all times in male athletes under WADA S2.2.1.
  • other: Stop for signs of severe allergy or a serious pump-site infection.

Interactions

  • Testosterone and other sex hormones: These can alter LH/FSH response. Intermittent gonadorelin has not been shown to preserve fertility during TRT.
  • Glucocorticoids: Can affect pituitary gonadotropin secretion and diagnostic interpretation.
  • Prolactin-raising drugs: Dopamine antagonists can blunt the GnRH response through higher prolactin.
  • Spironolactone, methyldopa and digoxin: Can alter measured gonadotropins and test interpretation.
  • Other GnRH medicines: Other drugs acting on the same axis can suppress or change response; they are not dose substitutes.
  • Clomiphene with ovulation induction: A historical hyperstimulation case also used clomiphene; combination monitoring matters.

Product quality risks

  • An FDA recall included compounded gonadorelin because of sterility concerns.
  • A June 2026 FDA warning identified an unapproved gonadorelin product. A research label does not establish approval.
  • No representative assay survey of online gonadorelin vials was found.

How Gonadorelin works

Gonadorelin tells the pituitary to release LH and FSH. These hormones signal the testes or ovaries. A pump copies the body’s repeated pulses; the timing is part of the therapy.

  • Pituitary GnRH receptor: Releases LH and FSH when responsive pituitary cells are present.
  • LH/FSH signaling: Supports sex hormones, follicle growth and sperm production.

Half-life: About 4 minutes in the UK product information.

Gaps: No controlled trial of twice-weekly gonadorelin preserving sperm or pregnancy rates during TRT was found. The 51-study count is a conservative review-based lower bound, not a complete census. Original U.S. label schedules were not retrieved; UK diagnostic schedules are not labelled as FDA/EMA protocols. Forum commonness describes sampled discussions, not measured population prevalence. No same-route, same-frequency human ceiling was found for intermittent TRT shots; pump doses cannot define it. Community vial stability is unknown; UK diagnostic handling cannot be transferred to a different formulation. Cognition benefit beyond the small Down syndrome pilot remains unknown.
